Health insurance typically covers most doctor and hospital visits, prescription drugs, wellness care, and medical devices. The cost of your health insurance premium is based on a combination of your copay, coinsurance, deductible, and maximum out-of-pocket costs. These plans will cover you locally in the US and usually do not provide coverage outside of your state and will not cover you outside of the US.
